Le logiciel n\u00e9cessaire a \u00e9t\u00e9 d\u00e9velopp\u00e9 \u00e0 l'Universit\u00e9 de B\u00e2le.<!--more--><\/h3>\n<div class=\"paragraph\">\n<p>Les m\u00e9decins ne doivent pas s'approcher trop pr\u00e8s d'une tumeur : \"Si nous coupons dans la tumeur, c'est absolument mortel pour le patient\", explique Michael Nogler, directeur adjoint et chef du service d'orthop\u00e9die exp\u00e9rimentale \u00e0 la clinique universitaire d'Innsbruck. En outre, les tissus environnants, par exemple les nerfs, ne doivent pas \u00eatre endommag\u00e9s. Si une incision est trop profonde, le patient risque de ne plus pouvoir bouger sa jambe ou retenir son urine apr\u00e8s l'op\u00e9ration.<\/p>\n<p>Pour tenir compte de ces facteurs avant l'intervention, on utilise les images bidimensionnelles traditionnelles de la tomographie assist\u00e9e par ordinateur (TAO) \u00e0 rayons X et de l'imagerie par r\u00e9sonance magn\u00e9tique (IRM) du patient. Alors que celles-ci sont normalement analys\u00e9es \u00e0 l'\u00e9cran, la RV permet d\u00e9sormais un examen beaucoup plus pr\u00e9cis.<\/p>\n<h3 id=\"3d-modell-mit-software-aus-basel\">Mod\u00e8le 3D avec logiciel de B\u00e2le<\/h3>\n<p>Des m\u00e9decins de la clinique universitaire d'orthop\u00e9die d'Innsbruck ont r\u00e9cemment pr\u00e9par\u00e9 la premi\u00e8re op\u00e9ration en Autriche dans l'espace virtuel. Le m\u00e9decin qui op\u00e8re met des lunettes VR et peut ainsi analyser toute l'anatomie du patient atteint d'une tumeur en trois dimensions. La nouvelle visualisation facilite ainsi les calculs visant \u00e0 lib\u00e9rer compl\u00e8tement le corps de la tumeur sans cons\u00e9quences graves.<\/p>\n<\/div>\n<div class=\"paragraph\">\n<p>La mani\u00e8re dont le logiciel d\u00e9velopp\u00e9 \u00e0 l'Universit\u00e9 de B\u00e2le proc\u00e8de n'est pas fondamentalement nouvelle : comme d'habitude, les donn\u00e9es du scanner et de l'IRM sont superpos\u00e9es en couches de mani\u00e8re \u00e0 obtenir des mod\u00e8les en volume ou des repr\u00e9sentations anatomiques. Le logiciel effectue ce rendu particuli\u00e8rement rapidement. Ce qui est nouveau, c'est la mani\u00e8re de les amener dans l'espace virtuel et de cr\u00e9er ainsi une nouvelle interface utilisateur avec laquelle nous pouvons travailler en collaboration\", explique Nogler.<\/p>\n<p>Les porteurs de lunettes VR ne sont donc pas les seuls \u00e0 pouvoir entrer dans la salle - l'enregistrement peut \u00eatre retransmis sur un \u00e9cran pour toutes les personnes pr\u00e9sentes. Selon le sp\u00e9cialiste, des experts internationaux peuvent \u00e9galement intervenir en direct lors de la pr\u00e9paration. L'analyse se fait de mani\u00e8re ad hoc et sans post-traitement manuel, comme l'exige souvent une analyse traditionnelle des donn\u00e9es d'imagerie par couches.<\/p>\n<h3 id=\"zoom-moeglich\">Zoom possible<\/h3>\n<p>Le grand avantage r\u00e9side surtout dans le fait que le m\u00e9decin peut planifier l'intervention dans des conditions presque r\u00e9elles et voir le corps du patient exactement comme il le verra pendant l'op\u00e9ration : en 3D. \"Regarder des informations en trois dimensions dans un espace virtuel est tout simplement beaucoup plus intuitif\", explique l'expert. Gr\u00e2ce \u00e0 un joystick, la visualisation peut \u00e9galement \u00eatre tourn\u00e9e et retourn\u00e9e - le m\u00e9decin peut en outre faire le tour de la repr\u00e9sentation et zoomer par exemple sur la tumeur.<\/p>\n<\/div>\n<div class=\"paragraph\">\n<p>La toute premi\u00e8re op\u00e9ration a \u00e9t\u00e9 pr\u00e9par\u00e9e \u00e0 grands frais plusieurs mois auparavant et a finalement \u00e9t\u00e9 r\u00e9alis\u00e9e avec succ\u00e8s. Lors de cette intervention risqu\u00e9e, une tumeur osseuse de la taille d'un ballon de football a \u00e9t\u00e9 retir\u00e9e \u00e0 un jeune patient au niveau de la hanche et des vert\u00e8bres lombaires. Depuis, cette m\u00e9thode est r\u00e9guli\u00e8rement utilis\u00e9e \u00e0 la clinique universitaire d'Innsbruck.<\/p>\n<h3 id=\"die-zukunft\">L'avenir<\/h3>\n<p>Selon Nogler, la prochaine \u00e9tape consistera \u00e0 int\u00e9grer compl\u00e8tement l'innovation dans le syst\u00e8me de radiographie de la clinique universitaire. A l'avenir, il serait \u00e9galement envisageable de simuler pr\u00e9cis\u00e9ment toute l'op\u00e9ration au pr\u00e9alable : \"Le logiciel ne nous permet pas encore de documenter et de simuler toutes les \u00e9tapes\", explique Nogler.
